The State of Ohio charges sales tax on all purchases and services. Mobility equipment
and mobility services are no longer exempt without a prescription or Mobility Enhancement
form signed by a Physician.
You may download this form to take to your family doctor before you purchase or
have products installed by M.C. Mobility Systems. Without this form you are responsible
for the sales tax on all mobility purchases and will have to file with the state
to receive a tax credit.
